Tisha B'av & Yom Kippur Pre-Fast Hydration: How to Prepare for the 25-Hour Fast

Every year around the High Holidays, the same pattern shows up in emergency departments across New Jersey: mid-afternoon on the fast day, someone arrives lightheaded, nauseous, with a headache that started at hour twelve and never let go. After 18 years of emergency medicine nursing, I can tell you the common thread — almost none of them planned their pre-fast hydration. They ate a big meal before sundown, had a glass of water with it, and assumed that was preparation.
A 25-hour fast doesn't work that way. Yom Kippur and Tisha B'Av are total fasts — no food and no water from sundown to nightfall the next day. Your body loses fluid continuously through breathing, sweat, and normal metabolism whether or not you're replacing it, which means the only variable you control is how hydrated you are when the fast begins. Pre-fast hydration — done deliberately, in the hours before candle lighting — is the difference between a fast spent in prayer and a fast spent counting the minutes until havdalah.
Why the 25-Hour Fast Hits Harder Than People Expect
Most "fasting" people are familiar with still allows water. The full Jewish fast days remove it entirely, and that changes the physiology completely. By hour 15–18, mild dehydration sets in for most adults: headache, fatigue, difficulty concentrating. Caffeine withdrawal stacks on top of it for coffee drinkers — often the real culprit behind the classic Yom Kippur afternoon headache. By the final hours, many people are functioning at a fraction of their normal capacity, on the one day of the year that asks for the most focus.
The traditional advice — drink extra water the day before — helps, but it has a ceiling. Drink a large volume of plain water in a short window and much of it passes straight through you, taking electrolytes along with it. What your body actually needs going into a long dry fast is fluid and the sodium, potassium, and magnesium that let your tissues hold onto it.
What a Pre-Fast IV Drip Does Differently
Our Yom Kippur & Tisha B'Av pre-fast drip delivers a full liter of balanced electrolyte fluid directly into your bloodstream — 100% absorption, no gastric limit, no racing to the bathroom every twenty minutes before Kol Nidre. Added B vitamins support energy metabolism through the long day, and magnesium supports the muscle and nerve function that dehydration degrades first.
The infusion takes about 45 minutes and is completed well before sundown, so it fits cleanly within halachic requirements — you enter the fast having consumed nothing after candle lighting, simply better prepared for it. Every infusion is administered by a registered nurse under the oversight of our Medical Director, Nicholas Luke, MD.
Timing Your Appointment Around Sundown
We schedule Yom Kippur and Tisha B'Av appointments backward from candle lighting in your town — the infusion finishes with comfortable margin before the seudah hamafseket (or the final meal before Tisha B'Av). Book a mobile visit and our nurse comes to your home while you prepare; families and groups fasting together often book back-to-back appointments in one visit.
Tisha B'Av and the Other Fasts of the Year
Yom Kippur gets the attention, but Tisha B'Av is the same 25-hour total fast — in the heat of July, which makes hydration preparation arguably more important, not less. The minor fasts (the Fast of Gedaliah, the Tenth of Tevet, the Fast of Esther, the Seventeenth of Tammuz) run dawn to nightfall rather than a full 25 hours, but they're still dry fasts, and a pre-fast infusion the evening before makes them noticeably easier. The same pre-fast drip is available ahead of every fast day on the calendar, with scheduling built around sundown each time.
Book Early — Especially for Yom Kippur
Yom Kippur is our single highest-demand fast day of the year, and appointment slots in the pre-sundown window are finite: every booking has to finish before candle lighting, so the afternoon fills first. If you're fasting this year, book as early as you can — and if you're reading this before Tisha B'Av, the same applies this week.
