It is the single most common cause of acute gastroenteritis in US adults, it moves through households and offices fast, and the defining symptom is that the person cannot keep liquids down, which is precisely the situation where driving to urgent care is the worst possible option and a home visit is the obvious one.
Food Poisoning or Stomach Bug? The Timing Usually Tells You
People ask us this constantly, usually around two in the morning. The honest answer is that you often cannot know for certain without testing, and it rarely changes what you do about it. But the timing gives you a decent guess.
Food poisoning tends to arrive fast and alone. If you were fine, ate something, and were violently ill within one to six hours, that points toward a preformed toxin in the food. It usually hits hard, burns out inside a day, and nobody else in the house gets it unless they ate the same thing.
A stomach virus works differently. It takes roughly twelve to forty-eight hours from exposure to symptoms, so the meal you are blaming probably was not the culprit. It often comes with body aches or a low fever, it lasts one to three days, and the giveaway is that it spreads. If your kid had it on Tuesday and you have it on Thursday, that is a virus, not the leftovers.
Either way, the treatment for the miserable part is the same: stop the vomiting, put the fluid back, and let it run its course. That is what this infusion does. What matters far more than which one you have is whether anything on our emergency list above applies to you, because that is the question with a different answer.
Why a Registered Nurse Hangs This One
The Stomach Soother is not a vitamin bag. It contains ondansetron and famotidine, both of them actual medications, and ondansetron in particular has a known effect on heart rhythm at higher doses. The FDA removed the 32 mg single intravenous dose from the US market in 2012 for that reason, and current labeling caps a single IV dose at 16 mg. The doses used in this infusion are a small fraction of that.
The safety is in the screening, not just the dose. Before anything is started, your nurse reviews your cardiac history, any family history of sudden cardiac death, and your current medication list for anything that affects the QT interval, including common antidepressants, methadone, and certain antibiotics. If you have been vomiting long enough that your potassium or magnesium is likely low, that changes the plan too.
This is ordinary practice in a hospital and it should be ordinary practice in your living room. It is the reason a licensed registered nurse administers this infusion and screens you first, rather than handing you a menu and a bag.
When a Stomach Bug Needs a Doctor, Not a Drip
Most nausea is what it looks like: a virus, something you ate, a bad reaction to a medication. It passes in a day or two and the worst part is the dehydration. That is what this infusion is built for.
But nausea and vomiting are symptoms, not a diagnosis, and a handful of things that cause them are surgical. Appendicitis, a bowel obstruction, a gallbladder attack, pancreatitis, and in women of childbearing age an ectopic pregnancy can all start out feeling like a stomach bug. Anti-nausea medication works on all of them. It will make you feel better for a few hours without touching the actual problem, and those few hours are the reason we ask before we treat.
Call an ambulance or get to an emergency room, not us, if you have severe or sharply localized abdominal pain, a belly that feels rigid or board-like, pain concentrated in your lower right side, vomit with blood in it or that looks like coffee grounds, black or tarry stool, abdominal pain along with a fever, vomiting with no bowel movement and no passing of gas, vomiting that began after a head injury, chest pain, or any confusion.
